Athletic Participation Requirements

Participation in athletics is a great way to get involved at John Long Middle School. To participate in interscholastic athletics there are FOUR (4) items that are required on file in the JLMS Main Office. They are due in the office prior to the first day of practice, and are as follows:

This is the original up to date Physical Card that is signed by your physician within the last year.
Physical Examinations taken April 1 and thereafter is valid for the following two school years; physical examinations taken before April 1 is valid only for the remainder of that school year and the following school year.

This card includes information such as family doctor, emergency contact phone numbers, etc. which is used when the parent(s)/guardian(s) is not in attendance.

JLMS Physician's Release Form


This form needs to be signed by both the parent & the athlete and turned into the JLMS Main Office prior to participating.  By signing the form you are stating that you understand the importance of recognizing and responding to the signs, symptoms, and behaviors of a concussion or head injury.

JLMS Concussion Information Form

  $ 50 per sport.
  $ 100 cap per student per school year. (3rd sport participating in is not charged)
     ** Only checks will be accepted for the athletic sports fee.  Cash will no longer be accepted.  Checks should be made payable to JLMS and given to the Main Office.

NOTE: All cards and forms can be accessed at the above links and should be returned to the Main Office during registration week.  Athletic Fee(s) will be collected at the time of the sport taking place.
